Telehealth Consent

I consent to receive healthcare services from CuraNow Health through telehealth, which may include live video or telephone/audio-only communication, when clinically appropriate.
I understand that:
Telehealth allows me to receive healthcare when my provider and I are in different locations.
Telehealth has limitations because my provider may not be able to perform the same physical examination that would occur during an in-person visit.
My provider may determine that I need an in-person examination, laboratory testing, imaging, urgent care, emergency care, or another level of care.
Telehealth may involve technical, privacy, or communication risks despite reasonable safeguards.
Receiving a telehealth visit does not guarantee a diagnosis, prescription, medication refill, antibiotic, or other specific treatment.
I may decline or withdraw my consent to telehealth at any time without affecting my right to seek future healthcare services.
CuraNow Health is not an emergency service. If I have a medical emergency, I should call 911 or go to the nearest emergency department.
I may be asked to verify my identity and physical location at the time of my visit.
I consent to live video telehealth services.
I separately consent to telephone/audio-only telehealth services when clinically appropriate.
By signing or electronically accepting below, I acknowledge that I understand the benefits, risks, and limitations of telehealth and voluntarily consent to receive telehealth services from CuraNow Health.
Video and Audio-Only Services
Video Telehealth
Live, interactive video may be used when clinically appropriate. Your provider may determine that an in-person evaluation, laboratory testing, imaging, urgent care, emergency care, or another level of care is needed.
Telephone / Audio-Only Telehealth
Certain healthcare services may be provided through live telephone/audio-only communication when clinically appropriate and permitted by applicable requirements.
An audio-only visit may have additional limitations because your provider cannot visually assess you. Your provider may determine that video or an in-person evaluation is necessary before providing or continuing treatment.
Privacy and Confidentiality
CuraNow Health will take reasonable measures to protect the privacy and confidentiality of my health information during telehealth services.
The same laws governing the privacy and confidentiality of medical information generally apply to healthcare delivered through telehealth.
I understand that I should participate in telehealth visits from a private location whenever possible.
I should not submit medical or clinical information through CuraNow Health's general website contact forms or regular email. Medical information should be communicated through CuraNow Health's designated secure clinical platform or another method specifically directed by CuraNow Health.
